I was cleaning my Enfield last weekend when I discovered that it felt funny when I pushed a pipe cleaner down the fire channel with the nipple out. Tried to stick a pick down the hole and it would not go in very far. I had stopped pulling my barrel and cleaning it in my utility sink as folks has said it was bad for accuracy to pull the barrel. So I had been doing it just with patches and brushes and a breech scraper.
So, I stuck my cheap-o endoscope down the bore and saw this:

You can see the fuzz around the hole in the "breech face" where the pipe cleaner had been coming through the wall of coke.
At first from this picture I first worried that there might be a bullet impacted down there.
Next go at cleaning got me this far:

You can see my pick sticking through the fire channel.
Finally I pulled the barrel and used my breech scraper while the breech was submerged in my utility sink full of hot soapy water.

This has taught me to be a little more vigilant in my cleaning of the breech face.
