need help!! Single action starr problems

Hello need some help I am having some problems with my pietta single action starr. After firing a shot the hammer will not advance the cylinder. The hammer it self will not cock at all. It only happens when you shoot. you can cock it and cylinder will advance *fine until you fire a chamber. its not the cylinder stop (bolt) *it is out of the cylinder stop slot. it seem like the recoil is knocking the hand out of place. I replaced the hand spring and this did not help. the hand it self looks good. *

Any one have any advice??*
